Patient Identification (ID) Card

The links below contain a form that can be carried as a medical device patient ID card. The patient ID card form contains a space to write the patient name, implant date, implanting doctor name and phone number, and a space to write device information or place a product adhesive label. There is also other important patient guidance information provided on the ID card form so it is strongly recommended each patient receive and carry this information. Please select a language below.
